Bulwell and Top Valley Primary Care Network are funding a project to target financial issues amongst patients registered with GP practices within the network.

The Social Prescribing Advice Link Worker, Ali Javadi, can assist patients registered with the following GP practices:

• Leen View Surgery
   

• Parkside Medical Practice

• Rise Park Surgery

• Southglade Medical Practice

• The Practice St Albans

• The Practice Nirmala
   

Ali can help with a wide range of issues including:​

• Checking benefit entitlements, making applications and supporting appeals 

• Managing general debts and arrears

• Managing fuel debts and arrears

• Fuel and food poverty

• Referrals for support to get into work, training or volunteering

• Application for charitable grants

If you would like to see Ali, please ask your GP to make a referral. They will send the referral to the social prescribing team and someone will be in touch.
