Bulwell and Top Valley Primary Care Network are funding a project to target financial issues amongst patients registered with GP practices within the network.
The Social Prescribing Advice Link Worker, Amoa, can assist patients registered with the following GP practices:

Amoa can help with a wide range of issues including:

Checking benefit entitlements, making applications and supporting appeals

Managing general debts and arrears
-
Managing fuel debts and arrears
-
Fuel and food poverty
-
Referrals for support to get into work, training or volunteering

Application for charitable grants
If you would like to see Amoa, please ask your GP to make a referral. They will send the referral to the social prescribing team and someone will be in touch.
